Job Duties:

Triage – Coordinate with providers and nurses to resolve health concerns 

Integrated Care – Promote the delivery of care across the continuum of services

Care Gap – attainment of identified preventive and chronic disease gaps in care

Failed Appointments – Follow-up on cancelled/no show medical, dental appointments

Lab tracking - Monitor lab orders for medical services

Data - Prepare data on Referrals

Qualifications/Skills:

Current RN or LPN license in good standing with the State of Louisiana (Required)

Current Basic Life Support (BLS) (Required)

Prefer experience in ambulatory care setting

Employment Type: Full Time, Monday through Friday

Starting Salary Range:     LPN - $18.66 – $24.35 per hour; up to 3% annual increases

                                                RN - $50,230 - $71,240 per year; up to 3% annual increases
