REMOTE PATIENT MONITORING
This program allows our providers to monitor their patients' health from the comfort of the patient's home.
Our Remote Patient Monitoring team works directly with the patient's provider to help improve high blood pressure, diabetes or congestive heart failure. Once enrolled, the patient will be given a monitoring device specific to their health concern to take home.
For our hypertensive patients, we have self-monitoring blood pressure devices that connect right to any smart phone via the Vital Care app. The app sends readings directly to our remote patient monitoring team to review. The team will check-in via phone call or text to ensure prescribed medications are being taken, the device is being used properly and to make sure patients do not need medical attention.
In the app, patients can track vitals, message our remote patient monitoring team & keep track of medications!
