Provides oversight for all Early Childhood Development (ECD) Program activities, at all RKM locations, including clinical and fiscal responsibility.

Develop and manage high functioning and competent teams with a focus on patient centered outcomes driven care

Assist in identifying and developing ECD program goals

Coordinate patient and family enrollment process into ECD program and with community resources.

Attend community outreach events (sometimes on weekends)

Increase the number of children ages 0-5 who receive recommended developmental screenings.

Increase number of children and their families assisted with accessing appropriate follow up/ referral services to ensure appropriate interventions and that documentation is complete

Track, compile and analyze clinical performance and outcome data for ECD patients

Reporting for program outcomes and progress toward goals

Assist in program dissemination as patients age out

Provide direct patient care, including assessment, planning, intervention and evaluation via ECD program and in clinic, as needed.

Build and maintain a long-term patient base in the provision of comprehensive care. One such that sustains the position and that of the Primary Care Team as well as meets the community’s expectation of access to healthcare.

Gather and document pertinent information in a timely and systematic, skillful format

Differentiate normal from abnormal variations for all body systems

Order diagnostics, monitor completion, review and interpret results, conduct follow-up

Develop individualized problem lists and related treatment plan

Provide appropriate pharmacological management in accordance with clinical practice guidelines

Perform procedures in accordance with standards of practice and clinical practice guidelines

Collaborate with other members of the interdisciplinary healthcare team of PCPFHF 

Order referrals for on-site services and off-site specialist, monitor completion and outcome


QUALIFICATIONS/SKILLS:

Current Advanced Practice Registered Nurse (APRN), with Pediatric specialty, licensure in good standing with the State of Louisiana (Required)

Current Basic Life Support (BLS)

Prefer a minimum of two years’ healthcare experience

Proficient with computer applications, including operations, applications and EMR

Effective verbal and written communication skills
